English-language studies from the United States, evaluating postpartum opioid prescribing or use changes within eight weeks after birth, were included in the analysis, considering interventions started after birth. Independent reviewers screened abstracts and full texts, extracted data, and assessed study quality employing the Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) instrument, along with risk of bias assessments using the Institutes of Health Quality Assessment Tools.
A total of 24 studies were deemed eligible. Postpartum opioid use during inpatient hospitalization was the subject of evaluation in sixteen studies, while ten studies investigated interventions to curb opioid prescribing after discharge. Modifications to standard order sets and protocols for post-cesarean pain management were among the inpatient interventions implemented. In a majority of the studies, these interventions were linked to a substantial decrease in inpatient postpartum opioid use, with only one exception.
